Multisector plan for aging (MPA) is a roadmap that states can use to transform the coordination of services for older adults, people with disabilities, and caregivers. This infographic shares best practices for states in the process of developing or enhancing an MPA. The infographic is a product of the Multisector Plan for Aging Learning Collaborative, a CHCS-led learning community, made possible by The SCAN Foundation, West Health and the May & Stanley Smith Charitable Trust, that is helping states advance their MPAs.
